* Change cvsweb references to viewcvs (collaboration.html and footer.html)

* Add dependencies for /head.html, /footer.html, and header.html in current 
  dir to almost all html files (build.zsh)

* Add header.html (and use it) in directories that are missing it to satisfy 
  the new dependency
